For each issue:
- Conversation and recorded evidence
- Expected behaviour under the policy at that time
- Cause: source / retrieval / selection / guidance / tool / delivery / measurement
- Proposed correction and owner
- Candidate learning or policy exception
- Regression case and release path
- Due date, decision and downstream verification

Also check source sync gaps, repeated questions, failed handovers, unknown writes and new exclusions.
